2) THE STREET ADDRESS OF YOUR TATTOO STUDIO
3) THE TELEPHONE NUMBER AND WEB ADDRESS FOR YOUR TATTOO STUDIO
- YOUR SHIPPING ADDRESS MUST BE THE SAME AS THE BILLING ADDRESS ON YOUR CREDIT CARD.
